Privacy Policy

Novian Care helps Michigan families enroll in the MDHHS Home Help Program. To do that, we ask for some information about you and the person you care for. This page explains what we collect, why, and what we do — and do not — do with it.

Information you give us

When you fill out our contact form, we collect:

  • Your name, phone number, and email address
  • Whether you are looking for a caregiver or looking to be paid as one
  • Who you are caring for (for example, a parent or spouse)
  • The daily activities you or your loved one need help with
  • Any complex care needs you select, such as wound care or respiratory treatment
  • Your service-area ZIP code
  • How you heard about us
  • Whether we may leave you a detailed voicemail
  • Anything else you choose to write in the message box

About health information

Some of what the form asks for describes a person’s health — the care they need, and conditions that require complex care. We ask only because the Home Help Program requires it to determine eligibility. Please share only what is needed, and only about someone you are authorized to speak for.

Novian Care is not a hospital, clinic, or health plan, and this form is not a medical record. If you would rather not type health details into a website, you can skip those fields entirely and tell us by phone instead.

Information collected automatically

Like most websites, ours collects some technical information when you visit: the pages you view, roughly where you are connecting from, and what browser and device you use. We use Google Analytics for this, which sets cookies in your browser.

We record an event when the contact form is submitted successfully, so we can understand which pages lead people to reach out. That event does not include your name, phone number, email address, or ZIP code.

Our pages load fonts from Google Fonts, which means Google receives your IP address when a page loads.

How we use your information

We use what you send us to:

  • Call or email you back about your enrollment
  • Determine whether you and your loved one qualify for the Home Help Program
  • Prepare and submit the paperwork the program requires
  • Understand how well our site is working

What we do not do

We do not sell your information. We do not rent, trade, or hand it to data brokers, advertisers, or anyone else to use for their own purposes.

Who else touches your information

We use a small number of companies to run this site. They process information on our behalf and are not permitted to use it for their own purposes:

  • Netlify — hosts this website and delivers your form submission to us by email
  • Google Analytics — measures how the site is used
  • Google Fonts — serves the fonts the site displays in

Enrollment and your consent

Enrolling in the Home Help Program means information has to reach MDHHS, and the program requires a physician’s authorization of medical need. Where that is necessary, we act on your instruction and with your consent, and we will tell you what is being submitted and to whom before it happens.

We may also be required to disclose information where the law compels it — for example, a subpoena or court order.

How long we keep it

We keep what you send us for as long as we may need it to help you with the program. If you would like us to delete your information, email or call us and we will do it.

Your choices

You can ask us what we hold about you, ask us to correct it, or ask us to delete it. You can also tell us to stop contacting you at any time, and we will stop.

To opt out of Google Analytics across all websites, you can install Google’s browser add-on. Most browsers also let you block or delete cookies in their settings.

Children

This site is meant for adults. We do not knowingly collect information directly from children under 13. A minor can be the person receiving care under the Home Help Program, and in that case an adult provides the information about them.

Security

This site is served over an encrypted connection, and form submissions are transmitted to us encrypted. No website can promise perfect security, but we work to keep what you send us safe and limit who can see it.

Changes to this policy

If we change how we handle information, we will update this page and change the effective date at the top.
